This Community Engagement presentation provides an overview of how Palliative Care South East supports people with life limiting and terminal illnesses to live their best possible life at home and in their community. It explains what palliative care is, who it supports, and how our multidisciplinary team works alongside clients, families and carers with compassion, kindness and respect.
The session also highlights:
• the diverse communities we serve across Melbourne’s south east,
• the importance of advance care planning, how and when referrals can be made, and,
• the vital role our volunteers play.
• Attendees will learn how to access our services, where we are located, and,
• practical ways the community can get involved or support our work.
Ideal for residents, community groups, health providers and partners wanting to better understand community based palliative care and how it can make a meaningful difference at end of life.
