Support coordination

Support coordination is about someone helping you to implement your NDIS plan.

​

Outlook’s NDIS support coordinator will work with you, and your family, to understand and implement your new NDIS plan, assist you to choose and connect with service providers, link with the community and mainstream services, maintain the service agreements and help you to achieve your goals. Outlook provide support coordination in the Casey Cardinia region to adults aged 18+.

You can feel confident that Outlook’s support coordination operates independently of the other support services we provide. We'll work with you to choose the best services and service providers for you, not us.
